Community Health and Wellbeing Workers (CHWW's) are the link between residents and borough wide services, keeping up to date with organisations, charities and council services so people can receive the support they need. CHWW's are also here to be a positive role model, guiding people to make better health choices, helping them to voice their concerns and therefore building their confidence.
 
Main duties of the role:
 
  • Visit households monthly to check on resident's health and wellbeing needs, taking a proactive approach. We like to get the job done!
  • Regularly connect with each household, either in person or by phone, to update them with the progress of the issue they have confided in us with and to listen and discuss any further needs.
  • Act as an advocate to help households navigate the health and social care systems, access the right services, and overcome barriers. 
  • Collaborate with colleagues across health, Local Authority, and the community sector to provide integrated an approach.
  • Build supportive relationships with local organisations, community groups, and services to make appropriate and hopefully quicker referrals.
