Community health & wellbeing workers – Cambridge Road Estate

Community Health and Wellbeing Workers (or CHWW's) are local people who really care about the health of their community. They work directly with people in their area (Cambridge Road Estate, phase 4 and 5 of the redevelopment) to tackle health and wellbeing issues, by building relationships with local residents and therefore spotting any unmet needs in the community.
 
CHWW's help to improve the health and wellbeing of the people they serve. This includes linking people into organisations that can help with specific needs, raising awareness, community education, giving advice, and offering informal counselling and support.
 
We can be the link between health and social services and the community, helping to break down barriers so people can receive the support they need. We are also here to be a positive role model, guiding people to make better health choices, helping them to voice their concerns and therefore building their confidence.
 
Main duties of the role:
 
  • Visit households monthly to check on resident's health and wellbeing needs, taking a proactive approach. We like to get the job done!
  • Regularly connect with each household, either in person or by phone, to update them with the progress of the issue they have confided in us with and to listen and discuss any further needs.
  • Act as an advocate to help households navigate the health and social care systems, access the right services, and overcome barriers. For example, helping to fill out forms linked with benefits or health issues.
  • Collaborate with colleagues across health, social care, and the community sector to provide integrated an approach.
  • Build supportive relationships with local organisations, community groups, and services to make appropriate and hopefully quicker referrals.

Kingston Community Football

Kingston Voluntary Action and Parkside Football Club have partnered up to provide ‘Kingston Community Football’ (KCF)- a FREE Sunday football project for men 18+ of all abilities, in the borough of Kingston upon Thames.

 KCF aim to bring together the community through football. The sessions are delivered in a friendly manner, with any ability welcome. Coach Ruben will ensure to make you feel welcome.

Sessions takes place on Sundays,13.00 at Fairfield Recreation Ground, Kingston, KT1 2PY. The sessions do take place on grass so boots and shin pads are required to be worn.

The approach defines a target population cohort and identifies ‘5’ focus clinical areas requiring accelerated improvement. NHS England » Core20PLUS5 (adults) – an approach to reducing healthcare inequalities.

KVA have launched our ‘community health’ offer to enable us to achieve Core20 outcomes. Our Community Health project worker will deliver mini health checks and facilitate engaging health talks and information sessions, focusing on empowering individuals with the knowledge and resources they need to make informed decisions about their health.
