This workshop focuses on optimising "joined up2 integrated care for people with cardiovascular disease risk factors and mental health concerns.

We are inviting those supporting people with cardiovascular disease risk factors and mental health concerns to attend, such as:

• Primary care professionals (e.g., GPs, nurses, care coordinators)
• ICS affiliated staff and commissioners involved in service design or delivery
• Social workers and allied health professionals
• VCSE organisations working in partnership with health and care services
